Alternatives to PDE-5 inhibitors

Many men who have to use certain heartmedication (nitrates) cannot take Viagra, Cialis or Levitra, known as PDE-5 inhibitors. The combination would be dangerous.

For others who have irreversible damage to the bloodvessels or the nerves the impotence pills may not work.

The good news is that there are other methods to help men to achieve an erection.

Uprima

Uprima is a tablet (2 or 3 mg) for under the tongue with a quick resolution. The active ingredient is Apomorfine that works by stimulation of the hypothalamus.

Via the receptors D2 and the agent cGMP the muscles in the walls of the bloodvessels relax and more blood can flood into the cavernes in the penis. This will result in an erection.

The side-effects are more or less comparable with the impotencepills. But there is no dangerous interaction with the heartmedication (nitrates), although care must be taken for heartpatients as fainting has been reported.

The sublingual tablet dissolves under the tongue, where it immediately enters the bloodstream, causing an erection in sexually stimulated men in about 20 minutes.

Since a few years Uprima is not available anymore in Europe.

Muse

With the use of an applicator (see picture) a drug (alprostadil) can be inserted about 2½ cm. into the urinary tube (urethra).

Alprostadil relaxes the muscles in the erectile tissue of the penis allowing increased blood flow and creating an erection. Within 5 - 10 minutes the erection will start and may last 30 - 60 minutes.

Different doses: Muse is available in four strengths - 125 mcg, 250 mcg, 500 mcg and 1,000 mcg. The initial dose is selected by your doctor.

Vacuumpump

The system consists of a plastic cylinder which is placed around the penis. A little pump gradually withdraws the air out of the cylinder. As a reaction the penis increases in size and blood flows into the penis like in a normal erection.

The erection is maintained by a tension ring at the base of the penis. The cylinder is removed and a normal intercourse is possible.

Big advantage of this method is that there is no pill to be swollowed and thus no waiting time for the medication to take effect.

There are different models and sizes available from different manufacturers. The reports of users of the vacuum pump are mostly very positive.

Impotence injections

After three sessions with your doctor you can yourself with a fine needle administer a medication called Caverject (or tri-mix) directly into the cavernes of the penis.

With this method you achieve an erection within 5 - 10 minutes and it will last about
60 minutes.

Penile implants

There are two types of implants:

Silicon rods that take the place of the two big cavernes. The erection is permanent. The penis can be bent downwards to the body for more
ease and concealment.
In the place of the cavernes that otherwise fill itself with blood come inflatetable chambers that fill up with the help of a little pump that has been situated in the scrotum. After the intercourse the sterile fluid can flow back to a little reservoir in the belly and the penis regains its normal size.

According to a recent research (British Journal of Urology jan.2006) about the results of the procedure in 504 cases, 89 percent of the men can have sexual intercourse again and 80 percent state that they are satisfied with the result.
